APPRECIATED STOCK, BONDS OR MUTUAL FUNDS

Fill out this form to be returned to the Advancement Office. The Advancement Office must be notified of your transfer as our broker does not receive information about the donor, only the securities.
Fill out the attached form to be sent to your broker. The attached form gives your broker instructions about where to transfer your securities, including account name and numbers. Please Note: There are two “broker forms”, one for mutual funds, one for stock/bonds.
Once word is received that the transfer is complete, the Advancement Office will send you an acknowledgement of your gift.
